LDN 4U 14-17 Camden South
This new service supports young people age 14-17 and 18+ living in South Camden.
We provide 1:1 support in your own home and out and about in the community.



We offer tailored support for young people aged 14–17 and adults aged 18+, ensuring a smooth and positive transition into adulthood.
Our support includes:
• Transition support from children’s to adult services
• Support in the home
• Community access and social inclusion
• Developing daily living skills
• Promoting independence and confidence
• Consistent, trusted support workers
We provide support 7 days a week, with flexible arrangements designed around individual needs.
Our team works closely with families and professionals to ensure continuity, stability and a seamless pathway into adult services.

- Young people aged 14–17 in transition planning
- Adults aged 18+
- Individuals living in South Camden
How to make a referral
Referrals are made through the Camden Social Work Team.
Social workers can refer directly to LDN 4U Camden as part of transition planning or adult care support packages.
